This week marks Dementia Action Week – a national event that encourages people to take action to improve the lives of people affected by dementia.
Nottingham and Nottinghamshire CCG and ICS is working with patients, carers, health and care professionals and wider stakeholders to shape the future service provision for the Nottingham and Nottinghamshire Dementia Well Pathway.
To support this, a period of engagement is taking place between 17 May, 2021 – 16 July, 2021 to understand the experiences of all those living with Dementia. As part of this work we’re encouraging all those living with dementia as well as carers, health professionals and charities to share their views via survey. You can find the survey here.
There is a range of dementia support readily available to residents across Nottingham and Nottinghamshire. You can also find support on the Nottinghamshire County Council website and Nottingham City Council website.

Dementia support workers offer information and practical guidance to help you understand dementia, cope with day-to-day challenges and prepare for the future. They offer information to people who are worried about their memory and on-going support to people affected by dementia face to face, over the phone or in writing. Social Prescribing is available via your GP practice and supports patients and carers on a 1:1 basis. A social prescriber takes the time to listen to the priorities of the individual and establish a care plan to improve their health and wellbeing through connections with local opportunities in the community.
The team at the Nottinghamshire Carers Hub is available 9am-5pm, Monday-Friday to provide information, advice and guidance as well as health and wellbeing support. The team also offer drop in sessions, peer support groups, carers grants, signposting to other organisations and respite. The memory assessment service offers early diagnosis for people experiencing memory problems and dementia. Each team is nurse-led, working closely with doctors and other healthcare professionals. Although there is currently no cure for dementia, there are treatments which can improve symptoms or temporarily slow down their progression. For more information call 0115 876 0155 or click here.
