The NHS England long term plan is for social prescribing to be embedded within all Primary Care Networks as business as usual by 2024 providing more patients with an opportunity to access social prescribing.

This is your opportunity to share your views and experiences. We would like to hear about what has worked well and what areas need improvement, as we continue to develop the social prescribing offer. Have you or your community group members accessed social prescribing? Have you worked with a link worker on issues such as housing, debt, finances, practical support, or joining a new social group? If so, we would like to hear from you. By completing this survey, you will be helping to shape the strategic direction of social prescribing as it becomes embedded within our Primary Care Networks (PCNs).
