If you are a patient registered at Platform One Practice, you may have received a letter about changes happening at your practice later this year. Please do not worry, there is nothing for you to do and the letter will explain what is happening.
You may have been sent a letter which says that you will be transferred to a GP practice closer to your home address. Or, you may have been sent a letter which says you are being moved over to the new local provider still in Nottingham City Centre – Parliament Street Medical Centre.
Please read this FAQ document for further information. The FAQ is split into two sections depending on which letter you received – please read the section which is meant for you. If you want to register at a GP practice of your choice – please do not worry. You can do this as long as the practice covers the area you live. There is more information on this is in the FAQ.
